Advanced Olmstead: Transition from Nursing Homes to Community

COURSE DATES: June 9-27, 2008

ORIENTATION TELECONFERENCE FOR STUDENTS: June 5, 2008

PURPOSE: This course will provide participants with an in-depth perspective on assisting people with disabilities as they make the transition from nursing home to community. The course will address key phases of transition and provide useful information for working with people who experience several secondary disabilities that often have an effect on transition.

TARGET AUDIENCE: Center and SILC staff, managers, and executives, as well as board members. This is an intermediate to advanced level course.

COURSE OBJECTIVES: Upon completion, participants will be able to assist in initial planning for transition, arranging housing and community services, providing support during and after transition, and helping the individual create and maintain formal and informal support networks. Participants will also be able to help those with psychiatric disabilities, substance abuse issues, and other secondary disabilities.

FORMAT: Participants will read assignments in the online course manual, complete exercises assigned by the instructor, and interact daily with the instructor and other students in an online discussion forum. Students will have an opportunity to learn about and share best practices. Students can ask and receive responses to their questions from the instructor and their peers. Participants will devote an average of 3-5 hours per week to assignments and activities.

THE INSTRUCTOR: Helen Roth has had extensive experience in the independent living movement and has served as the director of a Center for Independent Living in Utah. She is a consultant to the 2005 CMS Real Choices System Change Grantees and was also the Co-Chair of the CMS Home and Community Based Services Resource Network. She is the immediate past Chair of the Board of Directors for the American Association of People with Disabilities.

ONLINE COURSES FROM ILRU: This online course is presented by the IL NET, the national training and technical assistance project for centers for independent living and statewide independent living councils. The IL NET is operated by the Independent Living Research Utilization (ILRU) Program at TIRR in partnership with the National Council on Independent Living (NCIL). Online course implementation is facilitated by the Interdisciplinary Training Division of the Center for Persons with Disabilities, Utah State University.

The mission of the IL NET is to provide training and technical assistance on a variety of issues central to independent living today--understanding the Rehab Act, what the statewide independent living council is and how it can operate most effectively, management issues for centers for independent living, systems advocacy, computer networking, and others. Training activities are conducted conference-style, via long-distance communication, webcasts, through widely disseminated print and audio materials, and through the promotion of a strong national network of centers and individuals in the independent living field.
