Hurricane survivor with a disability MUST register to be able to receive the full range of federal assistance. Registrants MUST make clear that they have a disability or chronic health care need.

Find Family Call Center at 1-866-326-9393 will accept family member calls. This toll-free number has been established as an information and support resource for individuals and families who believe they have a family member who may have perished due to the hurrican.

Advocacy Assistance for Disability Issues

Louisiana

The Advocacy Center is identifying post-hurricane needs for advocacy and legal assistance for people with all types of disabilities. For assistance or information and referral any where in Louisiana contact the Advocacy Center at 1-800-960-7705 or email: advocacycenter@advocacyla.org. They may also be able to assist with community placement (versus nursing home), accessible transportation, accessible shelters, subsidized housing, and access to emergency program and benefits for persons with disabilities.

Food Stamps

Louisiana

For information on receiving the food stamp benefit in Louisiana, information is available on the Department of Social Services Web site at http://www.dss.state.la.us or call 1-888-LAHELPU or 1-888-524-3578. Louisiana residents receiving food stamp benefits can use their electronic benefits cards at many large retailers including HEB, Kroger and Wal-Mart.

In Baton Rouge area and you need access to food stamps you can call 225-219-1500 or 225-922-3000

Texas

For information on receiving the food stamp benefit in Texas, call 2-1-1 or the information service at the Texas Health and Human Services office at 1.888.312.4567.

UnitedHealthcare has established a 24-hour toll-free crisis hotline at 1-866-615-8700, which is available to anyone in the Gulf Coast region affected by the storms, regardless of their health insurance provider. Available 24 hours a day, seven days a week for as long as necessary, individuals can call with questions about health, mental health, financial and legal concerns. This includes persons who have lost medications or who have a short supply of prescritions.

The Louisiana Housing Finance Agency (LHFA) in an effort to provide displaced citizens as much assistance and information as possible during their stay away from home has established a toll free number. Individuals can call 866-310-7971 between the hours of 8:00 am and 9:00 pm. Central time. Staff members will provide the latest information on housing availabilities, and various other points of contact to assist Louisiana residents.

San Antonio Housing Authority Emergency Response Hotline at 210-477-6789 can provide federal housing assistance for those seeking housing option.


Medical Transportation

Texas

TxDOT will be providing Medical Transportation services, including Medical appointments and Dialysis appointments. Call 1.877.633.8747 toll free for services. This call can be made from the emergency center if necessary.


Medications

National

NeedyMeds.com is a Web site designed to provide information about patient assistance programs which provide no cost prescription medications to eligible participants at http://www.needymeds.com/ or call 215-625-9609

Texas

If you have lost your insurance card, or don't even have empty pill bottles identifying the strength of medication you need, the Texas State Board of Pharmacy at 512-305-8000 announced it won't enforce regulations that forbid pharmacists from dispensing more than a 72-hour emergency supply of medication.


Personal Assistance Services

Houston

Those who need personal assistance and who were Medicaid recipients in Louisiana are eligible for a special Medicaid category for five months in Texas. After that period, if they remain in Texas they will be placed on regular Medicaid coverage and be enrolled in Managed Care. If the individual is on SSI and decides to stay in Houston, they will be enrolled in STAR+PLUS when they change their address with the Social Security Administration. For assistance call 2-1-1.


Suicide Prevention

National

The National Suicide Prevention Lifeline has information and resources for crisis center staff and members of the public concerned about persons who have been residing in the devastated Gulf Coast region. http://www.suicidepreventionlifeline.org/

Veterans Information

U.S. Deparment of Veterans Affairs has information for veterans looking to find out about:

· Receiving health care for veterans evacuated from the storm area;
· Receiving prescription drugs for veterans in the stricken area;
· Locating evacuated VA patients; and
· Any other health care questions for veterans in the area affected by the storm.

Houston

The Houston Veterans Administration hospital has established a triage area for those affected by hurricane Katrina. Veterans should call 713-791-1414. And, Veterans can also go to the VAMC Main Campus at 2002 Holcombe Blvd. in Houston and go to Triage to sign in.


2-1-1 Texas Information and Referral Network

The Texas Information at 2-1-1 is in place and available providing information and referral for those in need of relief from the hurricane at 1-888-312-4567.
