About the Position

POSITION: PEER MENTOR

A.     JOB DESCRIPTON

Responsible for the mentoring of Center participants toward accomplishment of individual goals and objectives as outlined in Individualized Independent Living Plans.  Responsible for developing plans and strategies in concert with program participants which maximizes the individual’s independent living.  To provide emotional support and serve as positive role model to program participants.

B.     QUALIFICATIONS/BACKGROUND REQUIRED:

  •       Bachelor’s Degree in human services field.
  •       Experience can be substituted for some education.
  •       Courses in mentoring and case management.
  •       Experience in areas of consumerism and self-advocacy.
  •       Having a direct experience with disability.

C.     ESSENTIAL JOB FUNCTIONS:

  •        Individual peer mentoring.
  •        Developing Individualized Independent Living Plans with participants.
  •        Maintain accurate case notes and participant records.
  •        Reporting monthly statistics of peer mentoring services delivered.
  •        Participation in community education presentations as required.
  •        Working with DARS Fee for Service participants.
  •        Assist with Life Skills Classes targeted toward youth
  •        Serve as a positive role model for participants.

Starting Salary range: $27,727.00 -$29,064.00 Annually
