Service Coordinator-independent Living Social Support Specialist, Endependence Center, Inc., Norfolk, VA

About the position

The Service Coordinator level position is the position at the Center that works directly with Center participants and, as required, with family and other service providers to assist them in achieving their independent living goals. Service Coordinators provide a wide range of activities which could include counseling, mentoring, case management, skills training and other one-on-one services needed as it relates to public health, transition and diversion from high-risk congregate settings, access to services that will promote community living, and activates that address social isolation, social determinants of health and other activities that promote the well-being of people with disabilities. The Service Coordinator also performs services in the community as required which could include activities such as technical assistance, outreach presentations, community training and systems advocacy. The Services Coordinator position has a variety of titles based on the funding source regulations and specifications. The contract deliverables of the funding source that funds the program under which the Service Coordinator is employed determines the job title and the specific job duties.

QUALIFICATIONS/BACKGROUND REQUIRED:

  • Human Service degree is preferred; however, experience can substitute for some education.
  • Requires strong knowledge of disability service resources and issues.
  • Personal experience with a disability is preferred either by the coordinator or with a family member, however, it is not required.
  • Experience working in human service field and in a counseling or case management position preferred.
  • Strong interpersonal and communication skills are essential.
  • Other qualifications may be required by the funding source that funds this Service Coordinator position.

ESSENTIAL JOB FUNCTIONS:

  • Assist participants to explore, identify and establish personal independent living goals and to develop an individualized plan of action steps needed to accomplish these goals.
  • Document interaction with participant and record services provided.
  • Enter service units into required data collection system.
  • Serve as a positive role model and be willing to share personal experiences with similar life experiences.
  • Coordinate with other family members and service providers as needed at the discretion of the participant.
  • Attend all staff meetings and staff trainings as required.
  • Provide Information and Referral services as needed.
  • Work effectively in the “team approach” and consumer-directed philosophy of services.
  • Participate in any direct service and community service required of this position of the funding source.

HOURS/SALARY: Full-Time Position; Salary Range $15-$17.50
