2022 Recipients of The John A. Hartford Foundation Business Innovation Award Announced

The Aging and Disability Business Institute is proud to have presented Direction Home Aging & Disabilities (DHAD) in Uniontown, OH, with The John A. Hartford Foundation 2022 Business Innovation Award. This award honors the contract between Medical Mutual of Ohio and the 12 Area Agencies on Aging in OH with DHAD serving as Network Lead Entity. Annually, this program serves over 3,100 members across the state through home and telephonic visits, medication reconciliation and medication discrepancy resolution.

Runners up for the award include the Maryland Living Well Center of Excellence Division of MAC, Inc. in Salisbury, MD, which was recognized for its evidence-based chronic disease self-management programs, and Mid-America Regional Council, Department of Aging and Adult Services in Kansas City, MO, which was recognized for its partnership with Blue Cross Blue Shield of Kansas City.
