ACL’s Social Care Referrals Challenge

The Administration for Community Living’s (ACL) Social Care Referrals Challenge is an opportunity to create and modernize health information technology (IT) solutions that help foster connections between health care systems and community-based organizations (CBOs) that provide holistic health and social care for older adults and people with disabilities. A total of $500,000 in cash prizes will be awarded across three phases of the challenge for teams that develop interoperable and scalable technology platforms for health care organizations and community-based social services providers to share data, track referrals and report outcomes. Register for the Challenge and submit a Phase 1: Concept & Design proposal by December 14, 2020.
