Discharge Planning During the COVID-19 Pandemic Resource

This new resource provides guidance to those who are conducting discharge planning for adults with disabilities after COVID-19 treatment, helping them to reconnect with the supports and services they need after acute care. This resource highlights the Aging and Disability Networks as key partners in facilitating care transitions.
