Community-based organizations (CBOs) play an important role in the JAHF-supported Age-Friendly Health Systems and Age-Friendly Public Health Systems movements, which are helping spread age-friendly care that addresses the unique needs of older adults.

The John A. Hartford Foundation 2022 Business Innovation Award, presented by USAging’s Aging and Disability Business Institute, showcases CBOs that partner with health systems to improve the care of older adults. This year’s winner is Direction Home Aging & Disabilities in Uniontown, OH, for its work supporting older adults and individuals with disabilities as they transition from acute hospital stays back to their homes, resulting in reduced hospital readmissions and better outcomes for the people served. Runner-up winners include:
- The Maryland Living Well Center of Excellence, for their partnership with TidalHealth to improve the transitions of care that affect the ability of older adults and individuals with disabilities to age well.
- Mid-America Regional Council’s Department of Aging and Adult Services, for their work with Blue Cross Blue Shield of Kansas City to address the social determinants of health among people with disabilities and older adults.
