Report for Washington Health Home Managed Fee-for-Service (MFFS)

In 2012, Washington became the first state to partner with the Centers for Medicare & Medicaid Services when it launched the Washington Health Home Managed Fee-For-Service Model Demonstration. This demonstration targeted populations with the highest potential for health improvement and implemented an intensive care coordination approach that resulted in lower health care expenditures—and $30.2 million in Medicare savings in the demonstration's second year. With the mobilization of selected AAAs, Washington was able to produce better health outcomes by educating patients on how to develop enhanced self-management skills.
