Strengthening the Complex Care Ecosystem in Your Community

No one organization can solve the complex care needs of its whole community. A new 15-month learning collaborative from the National Center for Complex Health and Social Needs at the Camden Coalition of Health Care Providers, the Community Ecosystem Learning Collaborative, is designed to help communities develop and strengthen their own complex care ecosystems alongside peer organizations from across the country. Communities interested in this learning collaborative must apply as a team of three or more organizations. Applications are due January 15, 2021.
