Survey: The State of Complex Care

The National Center for Complex Health & Social Needs at the Camden Coalition is seeking input from the field about what complex care looks like right now in light of COVID-19. The National Center’s state of the field survey aims to capture the current state of complex care delivery from complex care professionals and programs from across the country. This survey is an opportunity to share your experiences with program design, COVID-19 response and the impact of increasing national attention on racial justice.
