Hartford Foundation Business Innovation Award recognizes community-based organizations that are improving health outcomes and quality of life for older adults and/or persons with disabilities, in their communities. Runners up for The John A. Hartford Foundation 2021 Business Innovation Award were Daybreak’s Rapid Response Case Management Pilot (RRCM) and the Area Agency on Aging & Disabilities of Southwest Washington’s (AAADSW) Community Paramedicine program, both programs that work with multiple community partners to improve the health and quality of life of individuals who are frequent users of emergency services.
