Tools for Life Health Literacy Study

Tools for Life is Georgia’s Assistive Technology Act Program. The purpose of this study is to gather insights about health literacy and assistive technology from people with disabilities and older adults to improve access to healthcare and health information. The insight gathered here will help educational materials be designed, advancing policy and informing health programs to assist individuals with disabilities and older adults along with their support networks, including family members, health educators, medical professionals, teachers and employers. By completing this anonymous survey, you consent to allowing your opinions, along with others, to be used in our research study.

Thank you for taking time to fill out this survey. Your feedback here is important. The survey should only take about 8 minutes.
