Center of Excellence to Align Health and Social Care's Center of Excellence Corner

The Center of Excellence to Align Health and Social Care (COE) is making progress toward the widely anticipated release of our Community Care Hub (CCH) RFP funded through a Cooperative Agreement with the Administration for Community Living. We anticipate release to occur in early- to mid-February. Watch this space for an announcement and link to the RFP!

We also want to highlight two developments from CCHs around the country. First, the Healthy Living for ME CCH in Maine recently published a video describing CCHs. Second, we welcome and applaud the Vermont AAAs for joining the CCH movement! The Vermont Aging Network Consortium (VANC) is a separate nonprofit entity consisting of all five VT-based AAAs.
