Multisector Plan for Aging Learning Collaborative: Request for Applications

The Center for Health Care Strategies (CHCS) is accepting applications from states for participation in the second cohort of a multi-state learning collaborative to advance Multisector Plans on Aging (MPAs). Applications are due May 1, 2023.

This second cohort will build on learnings from the first cohort of states that worked toward building MPAs, which are state-led planning processes that help transform the infrastructure and coordination of services for older adults and people with disabilities. MPAs help state leaders and stakeholders conduct high-level, cross-sector planning efforts to ensure that the needs of older adults, people with disabilities, and caregivers are met.

Up to 10 states will be invited to participate in this 12‑month collaborative. The initiative will help states to build on work already underway to better serve older adults and people with disabilities.

By participating in this collaborative, states will gain:

  • Access to peer states designing and implementing MPAs to share best practices and lessons.
  • Access to a network of subject matter experts, including individuals who have helped implement an MPA.
  • Technical assistance on how to build cross-sector buy in and facilitate a collaborative MPA development process.

The SCAN Foundation, West Health, and the May & Stanley Smith Charitable Trust provide funding to CHCS for this learning collaborative.
